WIN Georgia CME (Care Management Entity)

Our focus is to ensure children and adolescents from 6-18 get the care they need while supporting the family in the recovery process. To accomplish this goal, WIN Georgia utilizes evidence-supported approached to coordinate care, including: *High-Fidelity Wraparound *A youth-guided, family driven, strength-based, individualized, collaborative and culturally proficient approach to coordinate care across agencies and providers. *Intensive home and community-based services as alternatives to costly out-of-home placements. We provide Intensive Customized Care Coordination (IC3) Services to families in many counties of Northern Georgia (Specifically DBHDD Regions 1 & 3, and the northern sections of Regions 4 & 6).
